Patient Documentation

The traditional pathway / variance system is shown below, along with its limitations.

 

The Gateway Model uses a new documentation process which extends and adapts the pathway as the patient's condition changes.

 

Patients progressing as planned are documented on a "normal" day document:

 

 

 

When a delay is encountered the patient is documented on an "insert day" document which also captures the gateway variance data at the top.

 

 

This documentation process is easier, more robust, captures more data, and facilitates intra-team communication. This improved communication is essential for continuous improvement.
