The Gateway ReportThe Gateway Report is designed to facilitate analyzing gateway data for a clinical pathway. It uses a simple graphical format as acts a graphical executive summary for more detailed analysis of the data. Most of all is designed for the care team: Easy data entry, instant reporting, graphical format. Reading reports does not create change, acting on information does. A sample first page is shown below, for the full size version click here. The example is for a two gateway care process, such as neurosurgery or cardiac surgery. At the first gateway 15% of patients had a gateway variance with the reasons shown in the upper left histogram; 2% subsequently "fell-off" the pathway for the reasons shown on the lower left graph. At the second gateway, 50% had a gateway variance and nearly 20% fell-off the pathway. So what does this simple report tell us? One, the step-down unit phase is more problematic. Two, it tells us what the problems are in the step-down phase. The care team, or whomever needs to, focus on the identified step-down variances. What Now? From the Gateway Report the care team, and the data, can go in two different directions. Up the organization, combining with other performance measures along the way, as part of a balanced scorecard system and broader process improvement efforts. Or deeper into the causal structure of the problems the report has exposed. |
