(Briefly) Revisiting VMS Design Issues Gateway Documentation - Normal Days Gateway Documentation - Insert Days Pushing Variance Data Up The Ladder Summary and Conclusion |
Summary and ConclusionThis week we presented and discussed the Gateway Variance Model. The Gateway System is a more robust, powerful, seamless, and more effective variance management system. Gateways are the key patient care milestones, the activities or outcomes that determine the outcome of the patient's stay. A gateway variance indicates a major change in the patient's progression and the need for more attention from the care team and/or case manager if the patient is to meet the pathway's goals. The Gateway System also includes improved patient documentation. Insert days expand the pathway beyond the planned LOS when patient conditions dictate. Consolidating pathway documentation with progress notes and variance data collection greatly improves the care team's communication and problem solving. Gateway variances, by identifying the key variances, improve the efficiency of process improvement efforts. Further, gateway variances are highly compatible with balanced scorecard systems. Something to think about when dealing with Oryx requirements. Next week we begin our Tool Time sequence, starting with Process Thinking Tools. |
