Traditional Pathway / Variance DocumentationThe traditional clinical pathway document is the 2 x 2 matrix (see below). With interventions and outcomes on the vertical axis and time (usually in days but at times in "phases") on the horizontal. Variances are captured on the bottom, or the back, or on a separate document.
While this format provides a complete picture of the pathway, it has some serious limitations as a documentation tool. |
